Workers' Compensation Claims
If your orthopedic injury is work-related, you must provide the designated authorization letter given by your employer or workers’ compensation representative. We will need your claim or case number, in addition to the appropriate contact and phone number regarding the matter. You will also be required to provide a billing address as well.
Billing Information
Payment is typically requested at the time of your service, unless our orthopedic clinic is preferred by your insurance company or employer. In the event that your insurance does not cover the total amount due, you may be required to pay any additional charges that are not covered under your plan.
